Regarding "using open-source" (and not just for VoIP billing), you
_contribute_, either by writing the necessary code or by sponsoring
someone to do so.  Write your own, extend an existing package, or glue
together multiple existing systems. 

If I were looking to implement VoIP billing using open-source software,
(Blatant Self Promotion: I'm available for consulting) I'd start by
taking a careful look at Trabas to see if there was useful rating code
that could be ported/merged to use as a starting point, and then I would
implement the necessary rating code on top of Freeside.

Regarding "extremely inexpensive", unfortunately, development isn't
cheap.  On a more positive note, by sponsoring or contributing
open-source development instead of purchasing licenses, you could
amortize the cost over multiple customers.  10 customers paying $800
each could collectively sponsor a very significant amount of work.
