 What I see is that they are stored in attachment directories and referenced
both from the attachment index page and inlined in messages via lines like:
<img src="att-0000/02-TMXXXPIN.jpg" alt="TMXXXPIN.jpg">

 Are you suggesting that they shouldn't appear in the index page because
they are shown inline? It isn't clear to me whether that would be better.
The main reason for the current rule was that it was simpler to implement.
