Do you use the same autentication method for apache? i.e., "simple"? Do
you use the same protocol for apache? i.e., ldap or ldaps?

If you use the same credentials and the same protocol, i do not think
the problem is in hylafax. hylafax seems to correctly ask for binding.
LDAP server understand the request and refuse it.
