The fact that the jpeg docs weren't extracted point towards a problem in
gd.trm itself.  It may be caused by having more than one START_HELP in a
single .trm file.  

The order of appearance in the list will be fixed for the online .gih-type
docs by the sorting done by help.c.  I.e. the problem only exists in the
other documentation formats (printed, windows .hlp, ...) A better fix may
be to move this sorting into termdoc.c, where all output formats could
profit from it.

If such a dummy .trm is needed at all, move the jpeg help node into that
file, too.  And maybe the TERM_ENTRY section, too It won't have to contain
anything else, as long as its guaranteed that gd.trm will always be
included if this jpeg.trm is, by appropriate #if bracketing in term.h.
