kmail doesn't do anything with the passphrase, if you use the GPG plugin. 
everything is handled by the gpg-agent. 

kmail spawns cryptplugin calls gpg-agent spawns pinentry-{curses|qt|gtk}. The 
passphrase is cached by gpg-agent, and gpg-agent can be configured in 
~/.gnupg/agent.conf IIRC.
