It's difficult to know what you've done (and not done) from here.
Why don't you try to change your AxKit setup to use my fullguide.xsl and
fullguide-print.xsl instead of guide.xsl and guide-print.xsl and see if
it works before you start fiddling about with the files? I use them on
my AxKit setup. You did download and save the insert* files to the same
dir, didn't you?

I haven't has a single problem since I have used the setup mentioned in
a previous email.
