I've found the culprit and resolved it.  It turns out when you visit 
http://codex.gallery2.org/Special:NewFiles it tries to create thumbnails 
for existing images and the memory limits were set too low for 
ImageMagick to complete successfully.

As an aside, it was a real pain to backtrack this.  The way I did it was 
to write a Python script to use inotify to notice when a new file was 
created in temp, then take a snapshot of ps and the Apache server 
status.  This let me figure out the exact ImageMagick being run, the 
individual apache2 process that was calling it, and the URL that the 
apache2 process was handling at the time.
