Step by step breakdown:

TCP 665: TCPMDQS
TCP 2049: NFS (also on a udp port 2049, as you listed)
TCP 3306: MYSQL (also on a UDP port)

UDP 6: (unnasigned)
UDP 1024: (reserved)
TCP 2049: NFS (see above)

If anyone knows more about the UDP ports, jump in, but according to the iana
port assignments:

http://www.isi.edu/in-notes/iana/assignments/port-numbers

They are listed as quoted above. The problem with lists such as that, is
that they are out of date as soon as they are made.
