New description:

An unhandled exception is occurring when the FdoConnectionManager
destructor is invoked and the FdoModuleMap cleared.

A work-around seems to involve re-factoring the FdoConnectionManager
HMODULE map so that it stores non-const std::wstring objects. It is
unknown exactly why this change resolves the issue, but the change is seen
as safe and without any ancillary effect.
