Any device-mapper error messages in your kernel log like this?
device-mapper: dm-linear: Device lookup failed
device-mapper: error adding target to table

If so, it's most likely a bdclaim issue. Take a look at
http://evms.sf.net/install/kernel.html#bdclaim

If not, let me know and I'll have to think about it a bit more.
