Quick update for anyone following this problem.  After offline exchange of
information it was determined that there was a  problem with LVM on top of
a RAID5 array if and only if the LVM container (group) contained more than
~30,000 extents.

Fix has been merged into CVS tree and will be included in the 1.1 release.
