The 4th return slot has whatever the key is specified to return, usually
null.

If you mean the 4th passed slot, it is conceptually treated the same
whether the call was to a kernel key or any other key, namely a resume
key is generated. This resume key will be consumed when the kernel key
returns. But it is optimized, and no key is physically generated.
