I found the issue.  It turned out to be a permissions problem with the
ss_host_disk.php script.  It had was set to 700 and changing it to 755
fixed the issue.  i.e. 

It was (when broken)
-rwx------  1 webadmin apache 3781 Nov 13 11:30 ss_host_disk.php

Is now (working)
-rwxr-xr-x  1 webadmin apache 3781 Nov 13 11:30 ss_host_disk.php
