When a story is checked out, the version number does not increment until the
story is checked back in again.  In addition to being confusing when entering
notes (notes appear to be associated with a prior version until checked in, at
which point they switch to a future version) and documents, we think it could
be causing stability/caching issues when documents are checked in and out.  It
seems like a better solution would be to have the version increment when the
document is checked out, dropping away when a cancel checkout is performed, and
remaining when the document is checked in.
