It is possible to delete the primary category from a story, which then breaks
things horribly!

To do it:

Create a story in Category A
Add Category B to the story
Change the primary category radio button over to Category B and then check the
Delete box on category B and click the Delete button.

et voila, a single row in story__category with main = 0;

This give the error "There is no category associated with story ..." when you
try to preview or use it as a related story.

I could update the db directly to make it main = 1 but the query results seem to
be cached so it is necessary to restart bricolage to get that fix to take.
