Update & what I found out myself: 
In Apache 2 it is possible to use the following in httpd.conf:

AddHandler php-script .php
AddType text/html .php

In Apache 1.3, the mime type must be set to application/x-httpd-php for php
scripts to execute as the 1.3 compatible module doesn't allow the AddHandler
php-script syntax.
mod_include.c only allows files of type text/... when IncludesNoExec is set, though.

For my own config, I added a check for the php content type in handle_include in
mod_include.c (strncmp(rr->content_type, "application/x-httpd-php"...)

This is by no means a crucial problem, but it would be nice to have a config
option that controls which file types can be 'executed' when IncludesNoExec is
enabled.
